Parental Consent Form for Minors

*to be completed and brought at the time of the jump. Important: if the minor is 14 or 15 years old, the signatures of both parents are required. If the minor is 16 or 17 years old, the signature of one parent is required. It will also be important to bring a photocopy of an identification document from the parents who signed to confirm the authenticity of the signatures.
